Social prescribing helps you to explore services and other options of support to improve your health, sense of wellness and independence. Social Prescribing is for those who need a little extra help to navigate the services available to them in the community.
How does it work?
You can self -refer to a Social Prescriber Link Worker, who will contact you by phone to discuss your referral. They may arrange to visit you at your home or invite you to come and see them at the GP practice. This is a time-limited Service, tailored to you.
The Social Prescriber Link Worker is: A non-medical person who can help you find community-based activities and support. Someone who may be able to accompany you at the first visit to an activity, if you need help to build confidence. Someone who can link you to sources of specialist support to address your individual social need and desired outcome.
The Social Prescriber can help you explore activities and services in the community such as:
- social groups and activities, befriending and support groups
- specialist services such as for finance, support at home or for mild mental health issues
- opportunities available for volunteering and learning
- healthier and more active lifestyles
- ways to develop your own positive skills and interests
